APPOINTMENT SCHEDULING

Your dentist has informed you and referred you to our office for an oral surgery consultation and/or procedure. Except for patients with an emergency, the doctor must see you for a consultation first and schedule the procedure for a later date.

THE DAY OF YOUR APPOINTMENT

If you have any dental x-rays, please bring them with you. Otherwise, we will take them on the day of your consultation appointment. When you arrive, please be prepared to complete a personal and financial information form, and a comprehensive medical history. If you have medical problems you must give us the name and phone number of your physician. Also, please bring with you the names of all medications that you are currently taking.
