APPOINTMENT SCHEDULING
Your dentist has informed you and referred you to our Encino cosmetic dentist office for an oral surgery consultation and/or procedure. Except for patients with an emergency, the doctor must see you for a consultation first and schedule the procedure for a later date.
THE DAY OF YOUR APPOINTMENT
If you have any dental x-rays, please bring them with you. Otherwise, we will take them on the day of your consultation appointment. When you arrive at Smiler Dentistry Encino, please be prepared to complete a personal and financial information form, and a comprehensive medical history. If you have medical problems you must give us the name and phone number of your physician. Also, please bring with you the names of all medications that you are currently taking.
MINORS
Minors must be accompanied by a parent or legal guardian. There can be no exceptions.
FEES
Our fees are based upon the usual and customary community standard. Please feel free to discuss your treatment fees with Dr. Smiler or the management staff before beginning treatment.
BILLING AND INSURANCE
We appreciate your payment of fees at the time service is rendered. We accept cash, check, Visa or MasterCard. If you need special financial arrangements, our staff will assist you in establishing a mutually acceptable payment schedule.
As a courtesy, we will file forms with your insurance company. Each insurance company pays benefits according to the policyholder agreement. Unfortunately, we cannot be responsible for negotiating a disputed claim or for your company's delayed payments. Payment for services rendered are your responsibility. If you would like, we can pre-authorize your treatment plan so an estimate of your benefits from your insurance company can be determined. |
